Wellington Terrace Interdisciplinary Team

Wellington Terrace consists of 6 resident home areas (neighbourhoods) delivering care and service to 28 to 30 residents.  Neighbourhoods are supported by a care team made up of:

Nursing and Medical Services

  • Nursing staff are dedicated to providing a high standard of care and attention to the residents at Wellington Terrace. Our goal is to promote quality of life for our residents through maintaining their dignity and independence.
  • Residents and their families are viewed as an integral part of the care team and we encourage their participation in resident care and planning.

Staff in the Nursing Department

  • Our Medical Director and core group of physicians visit the home regularly and are available to our residents 24-hour/day to provide medical and palliative care. All residents are seen once or twice a month. 
  • Our Director of Care and Resident Care Manager are responsible for the overall nursing care overseeing and ensuring a high quality of care is provided to all residents.
  • Team Leaders (RN's) coordinate nursing care in the neighbourhoods and are a key contact for families and addressing any questions or concerns.
  • Registered Nurses are available 24 hours a day.
  • Registered Practical Nurses (RPN's) provide leadership on the Resident Home Area. RPN's administer medications and carry out physician ordered treatments.
  • Personal Support Workers (PSW's) are on duty 24 hours a day and provide the resident's personal care such as bathing, toileting, dressing and assisting with meals.
  • Terrace Aides assists the team to escort residents to the dining room, they assist residents with their meals and make beds.

Nutrition Services

  • The nutrition services department is responsible for the provision of quality, nutritious meals and snacks for to the residents of Wellington Terrace. Most of our food is prepared on site including fresh home baking. Our residents' individual nutritional care is supported by our in house Registered Dietitian and a team of dedicated staff committed to ensure special dietary needs are met.
  • Residents are always offered meal choices and can even participate with planning our regular and special event menus.
  • Families are welcome to have a meal with their resident in the home area guest dining room. As well, the fully equipped Wellington Room can be reserved in order that families/friends can bring in food or cook on site and enjoy some private time. Life Enrichment

Recreation Therapy

Therapeutic Recreation Services focus on individual needs while respecting freedom of choice. Residents may participate as much or as little as they wish. An opportunity to voice their interests and to make suggestions on programmes they would like to see offered is provided to residents through the Resident's Council monthly meetings. Families and friends are welcomed and encouraged to participate in activities. Programme activities are offered on weekends and evenings. Each Resident Home Area receives their own activity service calendar outlining the programmes and services offered that month.

A schedule is created monthly and posted in the resident home areas. Each neighbourhood features programmes and events that respond to the care level of the residents who reside in the area.

Social Services

The Social Services Worker is available for such services as, advocacy on behalf of the resident and families, supportive counselling, and assistance with adjustment to the new environment.

The Social Services Worker facilitates the Residents' Council meeting and Spousal Support Group.

Physiotherapy and Occupational Therapy

Physiotherapy services are provided at Wellington Terrace by qualified physiotherapist and physiotherapy assistant.  Residents may be referred for assessment and an individual programme can be established, which includes mobility prescribed exercises and equipment (walkers) to improve physical ability.  Occupational Therapy Clinic is held on Wednesday afternoons.  Referrals are made through the Registered Nurse.  Residents receive seating and positioning assessments; speech and language therapy services are provided on an as needed basis. 

Restorative Care

The Restorative Care Programme provides residents with individual, goal-orientated reactivation programmes which include assistance with range of motion, bed mobility and transfers, walking, dressing and grooming, eating and swallowing, communication, and prosthetic care.  Restorative Care is a valuable programme in assisting residents to improve or maintain their independent abilities.  The programme aims to enhance the individual's quality of life, respecting their dignity and promoting functional abilities while enhancing self-esteem and self-worth.  Restorative Care is provided by a trained Personal Support Worker and requests for service for individual residents may be made through the Neighbourhood Team Leader. 

Volunteer Services

Volunteers play a crucial role in enhancing every aspect of the quality of life for residents at Wellington Terrace.  Volunteer Services recruits, trains and assigns many volunteers to roles throughout the home.  Volunteers assist in activities such as crafts, visiting, discussion groups, shopping, picnics, bowling and hospitality at resident's meals.

Environmental Services

Members of the Environmental Services team are integrated into the interdisciplinary team in each Neighbourhood.  Members of the Housekeeping team participate in weekly team meetings in the Neighbourhood and contribute their ideas to care planning.  They ensure the resident home areas are clean, reducing the risk for the spread of germs.  Laundry services are available seven days a week and the Maintenance Team conduct preventative maintenance and are responsive to building needs.  A gardener maintains the outdoor courtyards throughout the home from May to October to provide a beautiful setting for visits and planned activities.  The Team's knowledge of the resident and their needs is supported through ongoing skill development.
