THIRD PARTY PHOTO, VIDEO AND AUDIO CONSENT AND RELEASE FORM

In exchange for sufficient consideration, which I have received, I hereby declare as follows:

  1. I am an authorized representative of the organization identified below (the “Releasor”).
  2. The photographs, videos, and/or audio recordings (collectively, the “Media”) provided to you by the Releasor were taken with the knowledge and consent of every person that was subject of the Media.
  3. The Releasor has the right and authority to, and does hereby, grant to the County of Wellington and its successors, assigns and licensees (collectively, the “County”) irrevocable permission in perpetuity to reproduce, broadcast, display, distribute, edit, publish and otherwise use without limitation all or any part of the Media in any manner or media, worldwide, and for any purpose, without notice or compensation to the Releasor or the subject(s) of the Media, along with the right to permit others to do the same. All moral rights in and to the Media have been waived with respect to the uses contemplated herein.
  4. TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, THE RELEASOR, ITS PARENT, SUBSIDIARIES, AFFILIATES, LICENSEES, AND EACH OF THEIR SUCCESSORS AND ASSIGNS (THE “RELEASOR PARTIES”) HEREBY IRREVOCABLY WAIVE ALL RIGHTS, CLAIMS OR INTERESTS WHICH AGAINST THE COUNTY THE RELEASOR PARTIES EVER HAD, NOW HAVE, OR MAY HEREAFTER HAVE IN RELATION TO THE USE OF THE MEDIA AS CONTEMPLATED HEREIN AND FOREVER RELEASE AND DISCHARGE THE COUNTY FROM LIABILITY UNDER SUCH CLAIMS . The Releasor agrees to indemnify the County, and the officers, directors, employees, agents, and licensees of the County in respect of any claims made by third parties in relation to any use of the Media as contemplated herein.
  5. I have read and understand this Consent and Release and agree to be bound by its terms.
